Near North Health operates community health centers in several parts of Chicago. The Winfield Moody Health Center is at 1276 N. Clybourn Ave. on the Near North Side. The North Kostner Health Center is at 1520 N. Kostner Ave., the Family Health Center at Oakwood Shores is at 3753 S. Cottage Grove Ave. in Bronzeville, and the Komed Holman Health Center is at 4259 S. Berkeley Ave. A Gage Park location on South Western Avenue handles WIC services only.
