Esperanza Health Centers

Esperanza Health Centers runs community health centers on the Southwest Side, including Esperanza Little Village at 3059 W. 26th St., which uses the Little Village Plaza parking lot. Other clinics are on South California Avenue, in Brighton Park and on West 63rd Street. Services include adult primary care, pediatrics, teen health, obstetrics and midwifery, behavioral health, psychiatry and substance use treatment. Esperanza accepts all Medicaid plans and many Medicare plans, and posts an income-based sliding fee scale for uninsured patients.
